Responsibilities

Develop QA goals and objectives. Coordinates and oversees the hospital QI plan development, review, revision, and implementation. Prepare agency and all pertinent QI information for monthly reporting to CNO. Develop department policies and procedures as they pertain to QA. Provide support for all quality improvement/risk management activities within the hospital. Monitor high-risk, high-volume, problem-prone situations in the hospital and mobilizing improvements in the hospital system to decrease the possibility of problems occurring. Also monitors situations that have or may cause harm to patients, families, visitors and hospital staff. Completes and submits all reports required to the regulatory bodies as required by the State. Maintaining excel reports, enforcing Q/A & Infection prevention measures. Must take admin call which includes being available to fill shifts in various departments as needed.

Qualifications or Skills Required

REQUIRED: RN licensed by the State of Texas. Bachelor’s Degree preferred. BLS required. Five years clinical experience minimum, 10 years preferred. Minimum of three years management experience. Must demonstrate the ability to coordinate and delegate to other members of the healthcare team. Mentor staff to enhance staff development and quality outcomes. Organized, self-directed, task oriented, and able to utilize Excel with proficiency. Requires intermittent walking, sitting and standing; some bending, stooping and reaching. Lifting up to 40 pounds and occasional heaving lifting in supporting or moving patients that may suddenly become weak or helpless.

Responsibilities

Conducts client intakes, focusing on diagnostic and functional evaluations, then makes recommendations to the primary care provider concerning clients’ treatment goals and plans. Advises primary care provider about which clients are better served at the primary care setting and which should be referred to specialty mental health facilities. Assists, to the extent feasible, in the clients’ community functioning by helping with public benefits, vocational rehabilitation, social support, housing, etc. Assists in the detection of “at risk” clients and in the development of plans to prevent worsening of their condition.

Qualifications or Skills Required

Must have a Master's Degree in Psychology, Mental Health Counselling, elated field from a CACREP accredited university. Must have behavioral health experience including individual, family and group therapy in a clinical setting. Must have effective oral and written communication skills. Requires intermittent walking, sitting and standing; some bending, stooping, reaching and lifting up to 50 pounds.

Under the supervision of the Director of Rural Health Clinics in conjunction with Clinic Supervisor and Chief Nursing Officer, provides nursing care in a clinic environment. Organizes a safe, clean setting for patients during their clinic visits. Utilizes appropriate equipment and resources and assists Medical Staff in the provision of clinic services and care.

Qualifications or Skills Required

Clinic Medical Assistant: High School graduate or equivalent and Medical Assistant certificate. One year experience as a MA, but willing to train. Must have oral and written communication skills. Computer skills required. Must be able to stand and/or walk for extended periods of time. Must be able to bend, stoop, reach, push, pull and lift up to 50 pounds. Provide occupational rehabilitative services to people under specific medical orders as part of a coordinated team. In coordination with OT, plan initial and subsequent treatment programs based on test findings. Participate in clinical conferences to share and coordinate the plan of care for patients. Complete all paperwork in required time specification. Perform duties under strict rules of confidentiality.

Qualifications or Skills Required

Graduate of accredited OTA program with a minimum of an associate’s degree or higher in Occupational Therapy Assistant. One year’s experience as a Certified Occupational Therapist Assistant preferred. Qualifications include current CPR license, Texas licensure and some computer skills required. Must be able to stand and walk for extended periods of time, bend, and reach when providing patient care. Pushing and pulling up to 300 pounds and carrying up to 50 pounds. Ability to lift or move patients when needed.
